I use the data collected to personalise the services offered and to send communications, including promotional communications. Personal information such as medical history and lifestyle are gathered to assess suitability for treatment, checking for contra indications, treatment with caution points and to form a basis for treatment planning, in accordance with Professional Body Federation of Holistic Therapists (FHT) Code of Conduct and Professional Practice 2015, Section 2, Assessing the Client’s needs.
5. Sharing Your Information
Personal information is not shared unless to support my care and treatment and only with your consent. There is a statutory duty to report notifiable diseases under the Public Health (Control of Disease) Act 1984 and the Health Protection (Notification) Regulations 2010. Any safeguarding issues raised or disclosed will be reported with no requirement for consent.
